Cagrilintide
Amylin Receptor Agonist
A long-acting amylin analog that complements GLP-1 therapy for superior weight loss and metabolic control.
Learn More: CagrilintideThe first dual GIP/GLP-1 receptor agonist, demonstrating superior weight loss outcomes compared to GLP-1 monotherapy.
Tirzepatide (brand names Mounjaro for diabetes, Zepbound for obesity) is the first approved dual GIP/GLP-1 receptor agonist, developed by Eli Lilly. It is a 39-amino acid synthetic peptide based on the native GIP sequence with modifications enabling GLP-1 receptor co-activation. The SURMOUNT-1 trial demonstrated a mean weight loss of 20.9% at the highest dose (15 mg weekly) over 72 weeks — substantially exceeding the efficacy of semaglutide in head-to-head comparisons. The SURPASS-2 trial showed superior HbA1c reduction compared to semaglutide 1 mg in type 2 diabetes. Tirzepatide represents a major advance in metabolic pharmacotherapy, with its dual mechanism addressing both the incretin axis and GIP-specific metabolic pathways that GLP-1 monotherapy cannot reach.

“The first dual GIP/GLP-1 receptor agonist, demonstrating superior weight loss outcomes compared to GLP-1 monotherapy.”
Tirzepatide is a “twincretin” that activates both GIP receptors (GIPR) and GLP-1 receptors (GLP-1R) with balanced potency. GIPR activation contributes to insulin secretion in a glucose-dependent manner, suppresses glucagon, and appears to have direct effects on adipose tissue including promotion of lipid storage and energy expenditure. GLP-1R activation provides appetite suppression, gastric emptying delay, and additional insulin secretion. The combination of both receptor pathways produces additive and potentially synergistic effects on weight loss and glycemic control that exceed GLP-1 monotherapy. Importantly, GIP receptor activation in the brain appears to complement GLP-1R signaling in the hypothalamus through distinct neural circuits, providing broader appetite regulation. Tirzepatide also improves insulin sensitivity in peripheral tissues through mechanisms that may involve adiponectin upregulation and reduction of ectopic lipid deposition.
Protocol Notes: Start at 2.5 mg weekly for 4 weeks, then increase by 2.5 mg every 4 weeks as tolerated. Maximum dose is 15 mg weekly.
